If you were unable to submit the registration form and wish to be billed separately, please complete the form below and we will email you an invoice which you can pay by debit/credit card/ or PayPal

If you don't have a title or position in the Division, just enter Member.

Please indicate which event package you wish to purchase.

We will send you an invoice by email from PayPal. You can pay by credit card if you do not have a PayPal account (you do not need to create a PayPal account).

 

The registration fee must be paid in addition to any other option except the Full Convention Package.
For members who are getting the Full Convention Package and the Installation Dinner, please indicate your meal choice. If you have specific diatary restrictions, please let us know using the comments section.
